Sample Action Plan

Goal
I want Sarah to increase her reading skills. Specifically, my goal is for Sarah to be able to read with less effort. I want Sarah to increase her understanding of the material she has read. I want Sarah's average grade to be a B- (or above).

Obstacles
1. Sarah has trouble understanding what she reads.
2. Sarah's instructor teaches reading in groups. Sarah reads better with individual attention.
3. Sarah’s instructor doesn’t understand how Sarah’s brain injury affects her reading skills.

Strategies
1. Have the school psychologist or reading specialist do a reading assessment to pinpoint Sarah’s challenges.
2. Provide teachers with relevant information about Sarah’s condition.
3. Learn some simple methods to help Sarah at home.
